If you wish to withdraw from the sales contract, this form should be completed and sent back together with the goods (new, without traces of use) to the service provider within 14 days from the date of delivery of the goods, directly to the following address: ul. Bielska 21, 43-356 Bujaków. Returns are made within 14 days from the date of receipt of the goods.
